Free Shopify Survey App: CSAT and How Did You Hear About Us

The app is free for every store, with unlimited responses. The previously published plan capped collection at 100 responses per month; that cap no longer exists. There is no trial, no card required, and no paid tier.

A second survey: "How did you hear about us?"

Until now the app had a single survey: a post-purchase CSAT rating with a follow-up text field. It now has a second, separate survey that asks buyers "How did you hear about us?".

The survey comes with five default answer options: Google Search, Instagram, Facebook, TikTok, and Friend or Family. All of them are editable; options can be rewritten, reordered, and new ones added. An optional "Other" choice gives the buyer a free-text field for a channel that is not on the list; it is off by default. Buyers can select more than one option, so a purchase driven by both a TikTok video and a friend's recommendation is recorded as both.

Results are shown as a pie chart of channel shares, with response counts and percentages, and each response stays linked to the order it came from. Since the answer comes from the buyer rather than from tracking, this also captures channels that carry no UTM parameters, such as a personal recommendation.

The survey is created as a draft; it does not appear on the thank-you page until the merchant sets it to Active. For longer option sets by industry, see How Did You Hear About Us? Options.

Six languages

The app now works in six languages. This covers the merchant admin, the surveys buyers see, and the App Store listing:

The admin follows the language set in the merchant's Shopify account, so there is nothing to configure.

The survey copy shown to buyers follows a different rule: it uses the store's primary language, not the merchant's admin language, because that copy is read by buyers. A German store run by an English-speaking merchant still shows its surveys in German. The app reads the store language at install and writes the default questions, scale labels, and thank-you message in it. All of that text stays editable afterwards.

The dashboard is translated as well: the CSAT score, the channel shares, and the responses listed next to their orders.
